Low SHBG should I be worried?

Here is my labs done Jan 2018 by NHS endocrinologist

What is your take on this? they are refusing to prescribe TRT based on the following, The only thing flagged is my SHBG?

Free T - 351pmol (198 - 619)
Testosterone - 11.1nmol/l (8.6 - 29.0)
Albumin - 46g/l (35-50)
SHBG - 9nmol (16.5 - 55.9)

I have to go for review appointment in 4 months time & retested if still low they want to do an MRI fossa?

Also I have been feeling like shit for years & been on every anti-depressant there is!

My serum test was 12.3 in Nov 2017 and 18 in jan 2018. Earlier in 2017 I tested 10.4. A similar result to 2005, also. So I know all too well what its like living with sub-par test levels! It sucks ass! Can't build or retain muscle for shit! No oomph to train! Strength fluctuates... It really sucks big time!

Free test is excellent but serum is below 12 so technically you could be a candidate for TRT. But the free test is very good so that maybe what will stop it in its tracks! Are you in the UK by any chance?

NHS? So you are in Britain. Basically, you have bob-hope of getting anywhere! Treat yourself and see if things improve!

Or go private! You can get a private script for testgel at pharmacy2u online with that serum test but its £66 a month 30 gels

There so much more to test! What about estradiol? Igf-1? LH, FSH? GP's and endo's in the UK are a joke when it comes to hypogonadism!

private endo diagnosed me in october 2017 with hypogonadism

tests done were

Testosterone 6.5nmol
LH 3.4
FSH 4.0
Prolaction 115

Was givin sustanon 250 for 6 weeks, worked the first few days then felt terrible, couldnt afford followup with private endo, though my doctor got me on waiting list for NHS was saw there in January & she did them tests, I requested testosterone shots but refused at this point? I have to go back in May for follow up, if still low MRI fossa? well thats what the letter here says anyway
 
Ah right. So they've DX'd you but won't treat you any longer? WTF??? As i say, the UK is light years behind in treating hypogonadism or recognising signs and Sx!

well your test is low but most wont prescribe trt at that level. you just have to find a more trt friendly doctor because there are plenty out there... you have to tell them how you are feeling, symptoms etc...
 
Thanks for the replies, with my SHBG being low is that something to worry about? it is flagged on my labs page here? I read that guys with low SHBG don't do well with TRT?
 
anytime you are out of range, you are going to get it flagged bro? they have to point it out to you?

Well, the abn low shbg is really boosting your free test. I doubt its anything sinister. Mine is in 50s which is shocking, but no one will help me!! SHBG binds to free test thus reducing your bioavailable test. So a low shbg is a good thing. I wish mine was that low! I doubt mine could get that low even with proviron!

As I say, pharmacy2u online will Px you testogel. Or just run your own TRT which is far cheaper! There's so much support about, that you can't go wrong! You can get sust for £25 for a 10ml vial which could last you 10 weeks (or more for TRT) so go for it! If you need a source, I'll offer one for UK :-)

With sust at TRT dose, when the prop reaches its half life, you hit a trough! Then its picks up again when the next ester hits. Sust is a strange one. The undeclonate (cant spell it lol) for me seems to give me the better gains and strength. Sust is an 'up & down' compound for sure!

ok so why is my serum t still low & I feel like shit if there is so much of this usable free T in my blood?
Albumin is quite high and test will also bind to this protein so that could be where a portion is going. Its such a complicated system. I don't have all the answers. These are questions for an endo!

So the bloods in OP are post sust course, yes? I'm not sure what effect it has on SHBG. Maybe it suppresses it? Test also converts to DHT. Whether that that conversion happens from free test or serum, I'm not entirely sure. That could be something to research :-)
